How would a Community Fund grant help you with your transition/medical care? Please specify what you need help with.
(For example: money for an ID change, help affording surgery, or a new wig or binder.)

How much do you need? Please give details about your budget.
(For example: $124 for an OR court order and $30 for a birth certificate, or $500 to pay for post-operative caregiving and $20 for a donut cushion.)

What resources will we have to work with?
(For example: Oregon Health Plan or other health insurance, a Care Coordinator, Paid Leave Oregon, or external sources of funding. Please let us know if you are eligible for OHP, SNAP, WIC, SSDI, a DSP, and/or subsidized housing.)

If you are applying for assistance paying for medical care at Transition Health, what could you reasonably afford as a monthly membership fee?
(Not applicable for those with OHP insurance, which will cover your care at Transition Health.)
