Transition Health Insurance Information

Oregon Health Plan

You can use your OHP insurance as usual at Transition Health, where it covers primary care, gender-affirming care, electrolysis, and more.
You can learn more about and sign up for OHP via the Oregon Health Authority.

Transition Health is in network with the following CCOs:
> Trillium in Lane County.
> Intercommunity Health Network (IHN) in Benton, Lincoln and Linn Counties.
> PacificSource Community Solutions in Crook, Deschutes, Jefferson, Klamath, Hood River, Wasco, Marion, and Polk Counties.

We can get a special agreement to take these CCOs:
> Cascade Health Alliance in Klamath County

If you have a different CCO, please contact us so we can see what we can do! We are still expanding our insurance network.

Private Insurance

We are now billing private (commercial) health insurance plans, usually obtained through work or ACA marketplace. More plans will be added to this list as they complete their contracting process.

Transition Health is currently in network with:
> PacificSource Health Plans
> Moda (Connexus, Synergy, and Affinity networks)

Coming Soon: United Healthcare/UHC, Regence Blue Cross Blue Shield

Medicare

In spring 2026, we will begin billing Medicare and the Advantage health insurance plans associated with the above insurance companies. We are currently awaiting processing of our application.
At the moment, we can bill PacificSource, Wellcare, and United/AARP Advantage plans if you have OHP as secondary insurance.


Direct Primary Care

Transition Health is a hybrid direct primary care provider for those without insurance or for those whose insurance network does not include our clinic. Your membership fee covers unlimited physician visits, telemedicine in all forms, and selected minor procedures.

If you have private health insurance, we keep that in mind when choosing where to send your tests and referrals. Our goal is to provide exceptional primary care, thus decreasing the need for external expenses, but when these are needed your insurance will apply to them as usual. Transition Health strongly recommends maintaining health insurance coverage in case of catastrophes. We can see patients without insurance, and we can also help you apply for insurance coverage.

Direct Care Membership Costs (not using insurance)

Primary Care for Ages 0-17 | $80 per month

Primary Care for Ages 18+ | $130 per month *** Attention: the waiting list is currently over a year long.***

Gender-Affirming Care only | $60 per month

  • Initial enrollment fee of $100 per person.
  • Discounts are available for additional family members.
  • Community Fund grants are available for affordable care for the queer community.
  • Donations to the Community Fund are gratefully accepted to help provide inclusive care.

Electrolysis Costs (not using insurance)

Face / Arms / Underarms / Legs | $100 per hour

Genitals / Chest / Back / Belly | $180 per hour

Numbing | $60 standard, then $30 per additional unit

  • If electrolysis is done for gender affirmation, health insurance can cover some (or all, for OHP) of the expense. This is only possible when you provide us with a letter of support from a behavioral health provider so that we can submit a pre authorization (PA) request, and we must wait for approval from your insurance company before we begin.
  • If desired for pain control, we have topical (lighter) and injectable (deeper) numbing options. We can discuss these at your consultation appointment.

Primary Care Goals & Process

Together, we can focus on holistic wellness, movement, nutrition, and restoration. Our goal is to help you control your chronic medical conditions and rapidly address any new concerns that may arise. We will first start with a consultation. When needed, we can then order tests and imaging, refer you to appropriate specialists, and be a central contact to coordinate your medical care across different locations and specialties.
